**Notice to Caitlin Pfeil’s Live Well 365 patients**

We are happy to announce that Caitlin will be seeing all of her patients at Leaves of Life beginning June 1, 2024.  All appointments can be in-person, over the phone or via Zoom. Her schedule at Leaves of Life allows for appointments on Mondays, Wednesdays, Thursdays and Fridays.

If LiveWell365 has routed you here to schedule and you have already seen Caitlin at least once, please schedule a 60 minute Follow-up.  You will only be charged for the time used.  (Caitlin bills in 5 minute increments with a minimum of 15 minutes). We will give you a call to get the rest of the information we need.  Please note: we cannot migrate any information from LiveWell365 to Leaves of Life without written permission. The abbreviated New Patient Paperwork that we send you will include this release.

If you have purchased a package at LiveWell365, it will be honored at Leaves of Life until it is completed.
